Student and Family Needs†
To help your child succeed in school, we want to partner with your family.
Here is a link to the Student and Family Needs Checklist (http://goo.gl/forms/EgDlEbiHTu). †Please
click on the link to complete the checklist if you would like someone to contact you regarding a
specific need. Some of the resources available to the school are dictated by outside providers,
therefore, services available and numbers allowed to be served, may be limited. † Please contact
Lisa Kallman, school counselor, at 402-715- 1320 or at lrkallman@mpsomaha.org if you have any
questions.†
The information you provide is confidential and made available only to school personnel and
resource providers that may be providing assistance.
Please inform us of your needs in order to better assist your family in as many ways as possible.
Additionally, completion of the checklist may assist MPS in developing future partnerships/resources†as well as programs to meet the needs of our student population.
